In your DOOM 3 directory, find "DoomConfig.cfg" (x:\Program Files\Doom 3\base). Open DoomConfig.cfg in Notepad.
Find the line:
seta image_cacheMegs "32"
Change it to 128 or 256 and see if it ups performance.

I guess thats like seta com_hunkmegs 448 in Quake3?
Default is 32, but you can safely put it at like 90% of your system resources.

Just tried that tweak..haven't noticed a difference (visually, speedwise that is). Didn't benchmark it though.

Actually thinking back, I first noticed that config option with Quake I. Made a world of difference pumping 28 of my 32mb of ram into it and running in DOS.

For ATi users this might be handy. Haven't had a chance to try it out myself.. but from reading reports it looks like this is a valid tweak.

Using the information from the link that Rotj posted, I was able to see a difference.
Before: timedemo demo1 = 48.6 fps
After: timedemo demo1 = 54.9 fps
800x600, ATI Radeon 9500
no AA, no AF. VSYNC off, all bells and whistles enabled.
P4 2.4 ghz, 1 gb ram
Windows XP

Ok I can build my own computer and all and set up everything correctly but I'm a video card noob. If I want the best performance with a GeForce 4 Ti4200 what do I need to turn on or off? I'm running in the lowest video setting with everything turned off and it's still choppy.
I have an Athlon 1800+
1 gig sdram

We also have a GeForce FX 5200 if I throw that in my rig and give Silv my Ti4200 will I get a big increase? Her comp is only a pentium 866mhz so I don't think the video card will make much difference.

The fx5200 will have a moderate slant over the ti4200.
But you should be running better than that. I only have a geforce 3ti500 and I can run it on high with 2X AA. (or is it 4?)
Average framerate of 30 or so (playable by my standards).
Im getting a 6800 in a few weeks, I cant wait. Been stuck with this "piece of shit" for too long. But, it has served me well enough.

It's weird because my system ALWAYS scores higher than any other system with my same specs...but Doom 3 is running like total crap, almost unplayable.
