It's a short film called "Rubber Johnny" by Chris Cunningham. He also did a bunch of other Warp videos for Aphex, Autechre etc.

I was truly disturbed...who thinks this crap up, and how much LSD did it take to come up with the concept? I can see it now, 4 hits of blottter, 6 grams of Meth, 1 oz of pot, some guy with a deformed head, a wheelchair, a camera, and a video editor

I was kinda freaked for the first 2 minutes. When the music started up I didn't like it anymore. They should have just stopped after the first couple of minutes. Then it might have exploded into something huge and controversial and semi-believable like "Alien Autopsy" or something. They stupidified it with the video and music.
